Indications
Gentamicin Ampoules is used in the following cases: The treatment of infections caused by susceptible bacteria includig; Staphylococcus sp. (coagulase negative), Enterobacter, Serratia sp., Klebsiella sp., Proteus mirabilis, Escherichia coli, Pseudomonas aeruginosa, and Proteus (indole positive) to be effective in: - Septicemia and bacteremia. - Infections of the kidneys and urinary tract. - Genital infections. - Respiratory tract infections. - Infections of bones, joints, soft tissue and skin especially infected wounds and burns. - Peritonitis. - Pelvic infections (including septic abortion). - Infections of the gastrointestinal tract. - Infections of the central nervous system (meningitis). - Eye infections; Gentamicin can be also administered by subcutaneous conjunctival in case of endophthalmitis
Warning & Precautions
- The nephrotoxicity and ototoxicity of gentamicin impose the following precautions for use: In case of renal insufficiency Gentamicin is used only when it is strictly necessary and the dose must be adjusted according to creatinine clearance. A medication supervision on renal function is necessary. - Repeated treatment and/or prolonged, particularly among the elderly should be avoided. - Patients should take care in case of intervention surgery.

Dosage
The dose will be adjusted according to the severity of infection, condition and age of the patient: In Patients with normal Renal Functions: - Adults : 3 mg / kg / day in 2 or 3 IM injections. - Children : 3 mg / kg / day in 3 IM injections (1 mg / kg every 8 hours). - Infants : 3 mg / kg / day, or 3 IM injections ( 1 mg / kg every 8 hours), serum antibiotic concentration should be monitoring. - Newborns (or premature) : 3 – 6 mg / kg / day in 2 IV injections infusion, serum antibiotic concentration should be monitoring. - Prophylaxis of post-operative infections in surgery; Gentamicin should be used with short duration. - Urological surgery : 2 – 3 mg / kg, usually in adult by administration of 160 mg single IV infusion dose of 30 minutes before the start of the intervention or by IM of 1 hour before the intervention begins. - Other surgeries: 1.5 mg/kg single IV dose of 30 minutes before surgery, or by IM of 1 hour before surgery, in association with imidazole or clindamycin. In Patients with Renal Insufficiency: - Dosage should be adjusted. - In hemodialysis : The initial dose is 1 mg / kg by IV injection. - In peritoneal dialysis : The initial dose is 1 mg/kg by IM injection.
Adverse Reactions
- Nephrotoxicity: - Gentamicin belongs to aminoglycosides for which renal insufficiency have been reported. - These side effects are most often related to too high dose or long-term treatment of renal impairment and in case of previous homodynamic disorders or associations with nephrotoxic products. - Ototoxicity: - They are related to too high dose, long duration of treatment, pre-existing renal insufficiency or associations with ototoxic products. - Minor allergic reactions (rash, urticaria) have been described and in these cases you must discontinue the treatment.
Contra Indications
- Hypersensitivity of gentamicin or aminoglycosides - Myasthenia
Drug Interactions
- The simultaneous administration of gentamicin with strong diuretics such as acid etacrynic or furosemide should avoided and some diuretics are themselves ototoxic. In case of intravenous diuretics may increase the toxicity of aminoglycoside by modifying the plasma and tissue concentration of gentamicin antibiotic. - Simultaneous local or systemic and/or subsequent other potentially neurotoxic drugs and/or nephrotoxoc such as cisplatin, cephaloridin, kanamycin, amikacin, neomycin - Polymyxin B, colistin, paromomycin, streptomycin, tobramycin, netilmicin, vancomycin, and viomycin should be avoided.
