Atropine Sulphate Indications

"Atropine sulfate ampoule is indicated in the following cases:

1) As an antisialagogue for pre - anaesthetic medication to prevent or reduce secretions of the respiratory tract.

2) To restore cardiac rate and arterial pressure during anesthesia when a sudden decrease in pulse rate and cardiac action occur.

3) To lessen the degree of artrioventricular heart block due to digitalis.

4) To overcome severe bradycardia and syncope due to a

hyperactive carotid sinus reflex .

5)As an antidote( with external cardiac massage) for injudicious use of a choline ester drug.

6) In the treatment of anticholinesterase poisoning from organo- phosphorus insecticides.

7) As an antidote for the “ rapid” type of mushroom poisoning.

Atropine Sulphate Warning & Precautions

"Atropine is a highly potent drug and due care is essential to avoid overdosage. Children are more susceptible than adults to the toxic effects of anticholinergic agents. Do not administer unless solution is clear and seal is intact. Discard unused portion. Atropine sulphate injection should be used with caution in all individuals over 40 years of age. Conventional systemic doses may precipitate acute glaucoma in susceptible patients, convert partial organic pyloric stenosis into complete obstruction or cause inspissation of bronchial secretions and formation of dangerous viscid plugs in patients with chronic lung disease

Atropine Sulphate Dosage

" ● Atropine sulfate ampoule may be administered subcutaneously, intramuscularly or intravenously.

Adults: 0.5 mg,range 0.4 to 0.6 mg.

As an antisialagogue it is usually injected intramuscularly prior to anesthesia.

Children: dosage ranges from 0.1 mg , in the newborn to 0.6 mg, injected subcutaneously 30 minutes before surgery.

● For bradyarrhythmias:

Adults: the usual intravenous dosage ranges from 0.4 to 1 mg every one to 2 hours as needed, up to a maximum of 2 mg may be required.

Children: the intravenous dosage ranges from 0.01 to 0.03 mg per Kg of body weight.

● In anticholinesterase poisoning from exposure to insecticides, large doses of at least 2 to 3 mg should be administered and repeated until signs of atropine intoxication appear.

Atropine Sulphate Adverse Reactions

"Dryness of mouth, blurred vision, photophobia, tachycardia and anhidrosis may occur with chronic administration of therapeutic doses.

Constipation and difficulty in micturation may occur in elderly patients.

Atropine Contra Indications

Atropine generally is contraindicated in patients with glaucoma , pyloric stenosis or prostatic hypertrophy except in doses ordinarily used for preanesthetic medication .

Atropine Sulphate Drug Interactions

"Increased antimuscarinic effects with nefopam. disopyramide, tricyclics, MAOIs, antihistamines and amantadine

Atropine delays absorption of mexiletine.

Absorption of Levodopa is possibly reduced .by antimuscarinics

.Antimuscarinics antagonize gastrointestinal effects of Metoclopramide and Domperidone.

Reduced effect of sublingual nitrates (failure to dissolve under tongue owing to dry mouth) with antimuscarinics

.Antagonism of effect with parasympathomimetics with antimuscarinics
