Indications
- Alpracid is indicated for short-term relief of anxiety disorders in which at least 6 of the following symptoms occur several times during 6 months or longer:
* Motor tension (trembling , twitching or feeling shaky, muscle tension, aches or soreness , restlessness, easy fatigability).
* Autonomic hyperactivity (shortness of breath or smothering sensation, palpitation or accelerated heart rate , sweating or cold clammy hands , dry mouth, dizziness or light-headedness, nausea diarrhea or other abdominal distress, flushes or chills , frequent urination , trouble swallowing or 'lump in throat'.
* Vigilance and scanning ( feeling keyed up or on edge , exaggerated startle response, difficulty concentration or 'mind going blank' because of anxiety, trouble falling or staying asleep , irritability).
- Alpracid is also indicated in panic disorders with or without agoraphobia, the diagnosis requires four such attacks within a four week period, or one or more attacks followed by at least a month of persistent fear of having another attack. the panic attack must be characterized by at least four or more of the following symptoms: dyspnea or smothering sensation , dizziness , unsteady feelings or faintness, palpitation or tachycardia , trembling or shaking , sweating, choking , nausea or abdominal stress , depersonalization or derealization, paresthesias, hot flushes or chills, chest pain or discomfort , fear of dying , fear of going crazy or of doing something uncontrolled.
- Alpracid is also prescribed for treatment of stress-related symptoms, unhappiness or minor physical diseases.
- Alpracid is indicated to relieve acute anxiety and related insomnia in children caused by fear (e.g. before surgery).
Warning & Precautions
* Treatment should be limited to the lowest possible dose for the shortest period. \
* Avoid prolonged use or abrupt withdrawal.
* Drowsiness may affect performance of skilled tasks.
* Caution is required in patient with impaired renal, hepatic or pulmonary functions.
* Caution is required in Porphyria, respiratory diseases, and muscle weakness.
* Alprazolam is not recommended for use in pregnancy or lactation.
* Safety and effectiveness of alprazolam in individuals below 18 years of age have not been established.

Dosage
- Anxiolytic disorders : initiating dose is 0.25-0.5 mg three times daily can be increased at intervals of 3 to 4 days to maximum 4 mg daily.
- In elderly patients, in patients with advanced liver disease or in patients with debilitating disease: the starting dose is 0.25 mg two or three timed daily ,can be increased gradually if needed and tolerated.
- For panic disorders: treatment can be initiated with a dose of 0.5 mg three times daily, depending on the response the dose may be increased at intervals of 3 to 4 days in increments of more than 1 mg per day.successful treatment require dose more than 4 mg daily but slower titration to the dose levels greater than 4 mg/day is advisable.
- For discontinuing therapy or decreasing the daily dosage: the daily dosage can be decreased by 0.5 mg every three days to avoid the danger of withdrawal.
Adverse Reactions
Side effects of alprazolam, if they occur, are generally observed in the beginning of therapy and usually disappear upon continued medication.
- The most frequent side effects are drowsiness and light headedness.
- Confusion and ataxia can occur especially in elderly also amnesia may occur.
- Muscle weakness and occasionally headache, vertigo, hypotension, dysarthria, tremor, visual disturbance, diplopia, increased salivation, change in libido, incontinence, urinary retention, GIT disorders, blood disorders, elevated billirubin and hepatic enzymes and jaundice may occur.
- other side effects: dizziness, seizures, hallucination and depersonalization.
Contra Indications
* Known sensitivity to alprazolam or other Benzodiazepines.
* Respiratory depression, acute pulmonary insufficiency or sleep apnoea syndrome.
* Sever hepatic impairment.
* Chronic psychosis.
* Acute narrow angel glaucoma.
* Alprazolam should not be used alone in depression or in anxiety with depression.
* Concomitant use with Ketoconazole and Itraconazole
Drug Interactions
- Opioid analgesics , Antihistamines , other Antipsychotics , Antidepressants , Alpha blockers , Moxonidine , Baclofen, Tizanidine, cimetidine and some antivirals such as Indinavir, Ritonavir and Amprenavir may enhance CNS depressant effect of Alprazolam.
- When antihypertensive drugs , Diuretics and Nitrates taken concomitantly with alprazolam hypotensive effect may be enhanced.
- fluoxetine and oral contraceptives may increase plasma level of Alprazolam.
- Rifampicin and Carbamazepine may decrease plasma level of Alprazolam also Theophylline may reduce effects of Alprazolam.
- Alprazolam may increase effects of Digoxine , Imipramine and Desipramine
- Alprazolam may antagonize the effects of Levodopa.
- Alprazolam may possibly increase or decrease plasma level of
- Phenytoin.
- Drugs demonstrated to be CYP3A inhibitors have profound effect on the clearance of alprazolam e.g. Nefazodone, fluvoxamine, propoxyphene, diltiazem, isoniazid, macrolide antibiotics(ex. Erythromycin and clarithromycin),grapefruit juice.
- Also sertraline, paroxetine, ergotamine, cyclosporine , amiodarone, nicardipine and nifedipine may interact with alprazolam.
