Mepecaine Indications
Mepecaine is the local anesthetic of choice. It is particularly recommended for patients in whom vasoconstrictors are contraindicated and in dental procedures not requiring lengthy pulpal anaesthesia.
Mepecaine Warning and Precautions
- Arterial pressure and pulse should be watched in emotional patients. - Inject slowly and in a continuous way, strictly away from the vessels, after repeated aspirations. - Incase of elevated doses, premedicate with benzodiazepine. - It should be used with caution in patients taking anticoagulants.

Mepecaine Dosage
As directed by the physician or as follows: Mepecaine can be applied for local infiltration, field block and nerve block. The site of deposition of the solution is determined by the dentist according to the area of operative intervention . Up to one cartridge may be used for dental block. The dose injected may be increased according to each case. The maximum dose is 6.6-mg/kg body weight. (Content of Mepivacaine Hydrochloride/ cartridge 54.0 mg ± 10%). An adult dose of Mepivacaine hydrochloride should not generally exceed 400 mg and the total dose in 24 hours should not exceed 1 gm.
Mepecaine Adverse Reactions
Like with other local anesthetics, secondary reactions may occur causing an abnormal sensitivity, allergy, overdosage or an erroneous technique. This type of reaction is impossible to be foreseen and it depends on the dose, rate of absorption and the state of the disease. The allergic reactions are characterized by cutaneous lesions (urticaria, oedema) or anaphylactic reactions. Local anaesthetics may produce systemic adverse effects as a result of raised plasma concentrations that occur when the rate of uptake into the circulation exceeds the rate of breakdown, for example after accidental intravascular injection, excessive or too rapid doses, absorption of large amounts through mucous membranes or damage skin, absorption of large amounts from inflamed or highly vascular areas. Systemic adverse effects include: CNS: restlessness, dizziness, paresthesia, tinnitus, blurred vision, nausea, vomiting, muscle twitching, tremor, convulsions. CVS: Myocardial depression and peripheral vasodilatation resulting in hypotention and bradycardia, Arrhythmia and cardiac arrest may occur.
Mepecaine Contra Indications
Mepivacaine hydrochloride as most other local anesthetics should be avoided in case of liver dysfunction, hypersensitivity to Mepivacaine and methylparaben, porphyria
Mepecaine Drug Interactions
- Antiarrhythmics: Tocainide, B-lytics, digitalic preparations, MAOIs, tricyclic antidepressants - Cimetidine - Antimyasthenics: The utilization of the neuronal transmission by the local anaesthetics may counteract the antimyasthenic effects on the skeletal muscles particularly if significant quantities are rapidly absorbed. The temporary adjustment of the antimyasthenic doses may be necessary in order to control the eventual symptoms of Myasthenia gravis: This effect is unlikely to occur in the doses used in odontostomatology. - Disinfectant solution containing heavy metals: risk of local irritation, swelling, oedema. Do not use for disinfection of mucosa before administration of the local anaesthetic.
